Before you start, have ready: your insurance card and a photo ID (you'll photograph both rather than typing the numbers), a list of your current medications, and the dates of any prior surgeries. Most patients finish in about 10 minutes.

I haven't received my link. What should I do?
Check your junk or spam folder first, and check your text messages, because the link can arrive either way. If it still isn't there, call us at (877) 777-8883 and we'll send it again to the email address or mobile number we have on file.
Do I need an app, a password, or an account?
No. The link opens your forms in whatever browser you already use, on a phone, tablet or computer. You'll confirm your date of birth to open them, and that's the only step that checks it's really you.
Do I have to finish everything in one sitting?
No. Your answers save as you go. If you stop partway through, open the same link again and you'll pick up where you left off.
What if I don't have a smartphone?
The link works just as well on a laptop or a desktop computer. If neither is easy for you, arrive about 15 minutes early and we'll hand you a tablet at the front desk with the same forms on it.
Can I fill out my forms before I book an appointment?

Where does my information go?
Straight into your chart. These forms are part of our medical record system rather than a separate form website, so what you enter isn't sitting with an outside company.
