1. Scope of this policy
This Website Privacy Policy describes how Texas Orthobiologics handles information collected through this website, texasorthobiologics.com.
It applies to this website only. Protected health information created or received in the course of your care is governed by the Health Insurance Portability and Accountability Act and by the practice’s separate Notice of Privacy Practices, which is provided to patients at the practice. This policy does not modify that notice.
2. Information you provide
This website offers a contact form and an appointment request form. When you submit one, the practice receives the information you enter, which may include your name, telephone number, email address, your preferred method of contact, and your message.
An appointment request may also include the reason for the visit, your preferred date and time, whether the request is for you or for another person, whether you have been seen at the practice before, your date of birth, and your acknowledgement of the practice’s communication consent notice.
Website forms and ordinary email are not secure channels and are not intended for detailed or highly sensitive medical information. Provide only what is reasonably necessary for the practice to respond, and discuss your history by telephone or in person.
3. Information collected automatically
Technical information recorded with form submissions
When a form is submitted, the website also records the Internet Protocol address and browser user agent of the device used, the page the form was sent from, and the date and time. This information is recorded to detect and block automated submissions.
Website analytics
This website uses Google Analytics 4 to understand how visitors find and use the site. It records pages viewed, how a visitor arrived at the site, general location derived from an Internet Protocol address, device and browser information, and certain interactions — submitting a form, selecting a telephone number, or reaching an appointment confirmation page. Google Analytics does not report your name or address to the practice. The practice has enabled Google’s email-redaction setting on this property. Information collected by Google Analytics is processed by Google under its own terms.
Web server logs
The practice’s hosting provider records standard server information for every request, including Internet Protocol address, the page requested, and browser type, and retains those logs under its own retention schedule.
4. Cookies
Cookies are small files a website places on your device. This website uses the following.
Functional cookies
This website stores your accessibility preferences — text size, contrast, and colour mode — so that they carry from page to page. These are stored on your device and are not transmitted to the practice or to any third party.
This website also sets one technical cookie that records that your browser has already been asked to refresh its cached copy of this website following a change of hosting provider. It contains no information about you and is not transmitted to any third party.
Analytics cookies
This website uses Google Analytics cookies (_ga and a property-specific _ga_ cookie) to distinguish visits and measure site usage, and cookies set by the hosting provider to measure traffic and maintain sessions.
Advertising cookies
The practice does not use advertising or cross-context behavioural advertising cookies. A Meta advertising pixel was present on the home page of this website until August 26, 2026 and has been removed. A cookie it set (_fbp) may remain in the browser of a visitor who came to the site before that date until it expires or is cleared.
Your control over cookies
The analytics cookies described above are set when the site loads. If you do not wish them to be set, you may block or delete cookies through your browser settings, or install Google’s opt-out browser add-on at tools.google.com/dlpage/gaoptout. Blocking cookies will not prevent you from using this website, but your accessibility preferences will not be remembered.
5. Service providers
The practice uses the following service providers in connection with this website. Some are contacted directly by your browser when a page loads.
- Its website hosting provider, for hosting, server logs, and traffic measurement
- Google, for website analytics (Google Analytics 4)
- Google, for the typefaces used across the site (Google Fonts), which receives your Internet Protocol address and browser details
- YouTube, in privacy-enhanced mode, for patient story and lecture videos, which receives your Internet Protocol address and, if you start a video, your interaction with it
- Microsoft 365, which delivers form submissions to practice staff by email
Google reviews shown on this website are retrieved by the practice’s own server. No information about you is sent to Google in order to display them.
6. How the practice uses this information
Information collected through this website is used to respond to your inquiry, to arrange or confirm an appointment, to contact you about your care using the method you requested, to operate and secure the website and prevent automated abuse, and to understand in aggregate which parts of the site are useful.
Information collected through this website is not used to make automated decisions about your care.
7. Disclosure
The practice does not sell personal information collected through this website and does not disclose it to third parties for their own marketing purposes.
Information may be disclosed to the service providers listed in Section 5 for the purposes described there, and may be disclosed where required by law, in response to lawful process, or where necessary to protect the rights, safety, or property of the practice or others.
If you become a patient, information you provided through this website may be added to your medical record. Information held there is part of your health record and is governed by the practice’s Notice of Privacy Practices and applicable law rather than by this policy.
8. Advertising
This website does not carry advertising and does not use advertising or retargeting technologies. A Meta advertising pixel was present on the home page until August 26, 2026 and has been removed. Meta may retain records of home page visits that occurred before that date; Meta’s own account controls govern that information.
9. Retention
Website form submissions, and the technical information recorded alongside them, are retained for twelve months and are then deleted. Records of blocked automated submissions are deleted on the same schedule. Deletion is automatic and runs nightly.
Server logs held by the hosting provider are retained under that provider’s own schedule.
Information that has become part of your medical record is retained for the period required by law and is governed by the practice’s Notice of Privacy Practices rather than by this policy.
10. Security
This website is served over an encrypted connection. Form submissions are stored outside the publicly reachable part of the server with restricted file permissions, and practice email operates on Microsoft 365 under the practice’s access controls.
No website or method of electronic transmission can be guaranteed to be completely secure, which is why the practice asks that detailed medical information not be sent through this website.
11. Your choices
- Analytics. You may opt out of Google Analytics using Google’s browser add-on at tools.google.com/dlpage/gaoptout, or by blocking cookies in your browser.
- Cookies. Your browser can block or delete cookies at any time.
- Communications. Tell the practice and it will stop contacting you, or change the method by which it does.
- Access, correction, or deletion. Write to info@texasorthobiologics.com. Information that forms part of your medical record is handled under the practice’s Notice of Privacy Practices and applicable law rather than under this policy.
12. Children
This website is intended for adults. The practice does not knowingly collect information from children under 13 through this website. An appointment request submitted on behalf of a minor by a parent or guardian is handled as part of that request.
13. Links to other websites
This website links to websites operated by medical societies, journals, manufacturers, financing providers, and other third parties. The practice does not control those websites and is not responsible for their content or their privacy practices. Review the privacy policy of any website you visit through a link from this one.
14. Changes to this policy
If this policy changes, the date at the top of this page will be updated. Material changes will be noted on this page.
